Load Conversational Form with Require.js/AMD

This example uses requirejs to init the conversation.
Atm. using local copy of Conversational Form, like when you use npm or bower to get a copy of Conversational Form.
Conversational Form 0.9.6 and above will support loading the interface from CDN.
